Does tirzepatide mess with my birth control?
Updated 2026-05-04
Yes, with the pill specifically. Tirzepatide slows stomach emptying, which can make oral contraceptives less effective. The Mounjaro and Zepbound prescribing labels say to use a backup non-pill method (condoms, IUD, implant, ring, patch, shot) for 4 weeks after starting and for 4 weeks after every dose increase. Semaglutide does not carry the same warning.
IfIf your only birth control is the pill and you're starting tirzepatide
Thenthen add condoms or switch to a non-oral method for 4 weeks
IfIf you just bumped from 5 mg to 7.5 mg
Thenthen restart the 4-week backup window from this injection
IfIf you're on an IUD, implant, ring, patch, or shot
Thenthen no change needed - those bypass the gut
Key facts
- Tirzepatide slows gastric emptying enough to reduce oral contraceptive absorption
- The Mounjaro and Zepbound prescribing labels both specify a 4-week barrier method window after start and after each dose escalation
- Semaglutide trials did not show the same effect on oral contraceptive levels
- Implants, IUDs, the ring, the patch, and the depot shot are not affected
- MHRA in the UK issued a similar safety alert for women on tirzepatide in 2025
