What to Expect
I got a lump, bruise, or red spot at my injection site - what now?
Updated 2026-05-03
Small lumps, bruises, and red spots are normal and not dangerous. A bruise means the needle nicked a tiny blood vessel. A red welt usually means an irritation reaction to the medicine or the bacteriostatic water. A small hard lump is often early scar tissue from injecting the same spot too often. Rotate sites, ice the area for 10 minutes if it stings, and the spot usually clears within a few days.
IfIf you got a small bruise
Thenthen ice it for 10 minutes, avoid that spot for at least a week, and inject elsewhere
IfIf you got a red itchy welt that lasts hours
Thenthen take an over-the-counter antihistamine (like Zyrtec or Benadryl), and check that you are using fresh bacteriostatic water
IfIf a hard pea-sized lump forms and stays
Thenthen stop using that area for 2 to 4 weeks - it is usually lipohypertrophy from too many injections in one spot
IfIf the spot is hot, expanding, throbbing, or you have a fever
Thenthen this could be infection - contact a doctor right away, do not wait it out
Key facts
- Bruising is more likely with thinner needles in busier capillary areas - the lower belly and outer thighs are usually safest
- Lipohypertrophy (fatty lumps) builds from repeated trauma to the same site and can blunt absorption by up to 25%
- An itchy welt is usually a local hypersensitivity to benzyl alcohol in bacteriostatic water - sterile water is an alternative
- Most bruises clear in 5 to 14 days without treatment
- Signs of real infection: warmth that spreads, throbbing pain, pus, or fever above 100.4 degrees F
- Cold compress immediately after injection reduces both bruising and irritation
